Manifest Wellness Spa operates as a Private Membership Association (PMA). Our wellness services are provided exclusively to members.
To receive services, visit the spa, or participate in any wellness offerings, individuals must first become members.
There is a one-time lifetime membership fee of $20.
Payment must be completed on-site during your first visit, before any service is provided.
By completing the form below, you acknowledge that you are joining Manifest Wellness Spa as a private member and agree to the terms outlined in the PMA Agreement and Liability Waiver.
Manifest Wellness Spa – A Private Ministerial Association
By joining Manifest Wellness Spa, a Private Membership Association (PMA), and/or any website or social media group organized or maintained by the Association, I agree to the terms and conditions of Manifest Wellness Spa’s PMA Membership Agreement as follows:
Our Association exists to provide members a private platform to conduct all manner of wellness-related activities, education, and support within the private domain. We operate under the protections acknowledged by the Universal Declaration of Human Rights (UDHR) and the Constitution of the United States, including freedom of association, freedom of speech, freedom of religion, and the right to assemble peacefully to support each other in achieving wellness and exercising our protected rights.
We believe these constitutional and human-rights protections guarantee our members the freedom to seek spiritual, emotional, physical, and mental wellness guidance, and the freedom to choose which members or practitioners they deem appropriate for personal advice or support.
Our members may choose any therapies or wellness practices they believe support their well-being, including holistic, energetic, and spiritual modalities. Manifest Wellness Spa is a private platform through which members may conduct business with one another under constitutional protections.
Membership may be terminated at any time if a member’s behavior is found to be disruptive, harmful, or contrary to the principles of the Association.
I understand that as a PMA, Manifest Wellness Spa operates outside the jurisdiction of federal and state agencies concerning complaints or regulatory oversight. Any grievances will be handled internally by the Association’s designated committee or tribunal.
I confirm that I am joining this Association freely and of my own will, and that I do not represent any government agency or regulatory authority. I affirm I have received enough information to determine that joining the PMA is the right choice for me. I understand I may withdraw my membership at any time, and that the Association may revoke my membership if necessary.
I understand that the products and services offered by Manifest Wellness Spa support the body, mind, and spirit’s natural ability to promote wellness. I acknowledge that Manifest Wellness Spa practitioners do not diagnose, do not prescribe, and do not perform medical treatment. Their services are not a substitute for medical or psychological care.
I understand that I should consult a licensed medical or mental health professional for any condition requiring such care and that the holistic services provided by Manifest Wellness Spa may complement—but do not replace—professional treatment.
I accept that the body may require multiple sessions to achieve relaxation or energetic balance and that all healing outcomes are individual.
Manifest Wellness Spa and its practitioners are not liable for my medical, emotional, or psychological conditions before, during, or after any session.
I take full responsibility for my well-being and for all decisions I make regarding my care. My heirs, guardians, legal representatives, and I hereby release and discharge Manifest Wellness Spa and its affiliates from any and all claims or liabilities connected to my participation in Association activities or wellness services.
For the safety of all members, I understand that security cameras are present in the waiting area and session room.
By signing below, I confirm that I have carefully read and fully understand this PMA Membership Agreement and Liability Waiver, and I agree voluntarily and without coercion.
